Administration

To get the best from your treatment, please read the following carefully:

The recommended dose of Valif Oral Jelly 20 mg is 10 mg taken as needed approximately 25 to 60 minutes before sexual activity. Based on efficacy and tolerability the dose may be increased to 20 mg or decreased to 5 mg. The maximum recommended dose is 20 mg. The maximum recommended dosing frequency is once per day. Valif Oral Jelly 20 mg can be taken with or without food. The mean terminal half-life of vardenafil in patients receiving Valif Oral Jelly 20 mg orodispersible tablets ranged between 4 – 6 hours. The onset of activity may be delayed if taken with a high fat meal. Always take this medicine with a large glass of water.

You should only take a Valif Oral Jelly 20 mg when you plan to have sex – it is not intended as a regular medication.

When vardenafil film-coated tablets are taken with a high fat meal (containing 57% fat), the rate of absorption is reduced, with an increase in the median tmax of 1 hour and a mean reduction in Cmax of 20%. Vardenafil AUC is not affected. After a meal containing 30% fat, the rate and extent of absorption of vardenafil (tmax, Cmax and AUC) are unchanged compared to administration under fasting conditions.

Valif Oral Jelly 20 mg can lower blood pressure, and combining it with alcohol (ethanol) may further increase this effect. You may be more likely to experience symptoms such as dizziness, lightheadedness, fainting, flushing, headache, and heart palpitations. You should avoid or limit the use of alcohol while being treated with Valif Oral Jelly 20 mg, and use caution when getting up from a sitting or lying position. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Grapefruit juice may increase the blood levels and effects of certain medications such as Valif Oral Jelly 20 mg. You may want to limit your consumption of grapefruit and grapefruit juice during treatment with Valif Oral Jelly 20 mg. However, if you have been regularly consuming grapefruit or grapefruit juice with Valif Oral Jelly 20 mg, do not alter the amounts of these products in your diet without first talking to your doctor or other healthcare professional. Contact your doctor if your condition changes or you experience increased side effects. Orange juice is not expected to interact.

Rare side effects

Warning

Valif Oral Jelly 20 mg is not suitable for everyone.

Patients should NOT take Valif Oral Jelly 20 mg:

  • If you are allergic to vardenafil or any of the other ingredients of this medicine. Signs of an allergic reaction include a rash, itching, swollen face or lips and shortness of breath
  • If you are taking medicines containing nitrates, such as glyceryl trinitrate for angina, or nitric oxide donors, such as amyl nitrite. Taking these medicines with Valif Oral Jelly 20 mg could seriously affect your blood pressure
  • If you are taking ritonavir or indinavir, medicines used to treat human immunodeficiency virus (HIV) infections
  • If you are over 75 years of age and are taking ketoconazole or itraconazole, antifungal medicines
  • If you have a severe heart or liver problem
  • If you are having kidney dialysis
  • If you have recently had a stroke or heart attack
  • If you have or have had low blood pressure
  • If your family has a history of degenerative eye diseases (such as retinitis pigmentosa)
  • If you have ever had a condition involving loss of vision due to damage to the optic nerve from insufficient blood supply known as non-arteritic ischemic optic neuropathy (NAION) If you are taking riociguat.This drug is used to treat pulmonary arterial hypertension (i.e. high blood pressure in the lungs) and chronic thromboembolic pulmonary hypertension (i.e. high blood pressure in the lungs secondary to blood clots). PDE5 inhibitors, such as Valif Oral Jelly 20 mg have been shown to increase the hypotensive effects of this medicine. If you are taking riociguat or are unsure tell your doctor

Prior to initiating any treatment for erectile dysfunction, it be should considered the cardiovascular status of the patient, since there is a degree of cardiac risk associated with sexual activity. Vardenafil has vasodilator properties, resulting in mild and transient decreases in blood pressure. Patients with left ventricular outflow obstruction, e.g., aortic stenosis and idiopathic hypertrophic subaortic stenosis, can be sensitive to the action of vasodilators including Type 5 phosphodiesterase inhibitors.

Serious cardiovascular events including:

  • sudden death
  • tachycardia
  • myocardial infarction
  • ventricular tachy-arrhythmia
  • angina pectoris

Cerebrovascular disorders (including transient ischaemic attack and cerebral haemorrhage), have been reported in temporal association with vardenafil. Most of the patients in whom these events have been reported had pre-existing cardiovascular risk factors. However, it is not possible to definitively determine whether these events are related directly to these risk factors, to vardenafil, to sexual activity, or to a combination of these or other factors.

Medicinal products for the treatment of erectile dysfunction should be used with caution in patients with:

  • anatomical deformation of the penis (such as angulation, cavernosal fibrosis or Peyronie’s disease)
  • in patients who have conditions which may predispose them to priapism (such as sickle cell anaemia, multiple myeloma or leukaemia)

The concomitant use of alpha-blockers and vardenafil may lead to symptomatic hypotension in some patients because both are vasodilators. Concomitant treatment with vardenafil should only be initiated if the patient has been stabilised on his alpha-blocker therapy.

Concomitant use of vardenafil with potent CYP3A4 inhibitors such as itraconazole and ketoconazole (oral form) should be avoided as very high plasma concentrations of vardenafil are reached if the medicinal products are combined. Vardenafil dose adjustment might be necessary if moderate CYP3A4 inhibitors such as erythromycin and clarithromycin, are given concomitantly.

Concomitant intake of grapefruit or grapefruit juice is expected to increase the plasma concentrations of vardenafil. The combination should be avoided.

Contraindications

The co-administration of vardenafil with nitrates or nitric oxide donors (such as amyl nitrite) in any form is contraindicated.

Valif Oral Jelly 20 mg is contraindicated in patients who have loss of vision in one eye because of non-arteritic anterior ischemic optic neuropathy (NAION), regardless of whether this episode was in connection or not with previous phosphodiesterase 5 (PDE5) inhibitor exposure.

Medicinal products for the treatment of erectile dysfunction should generally not be used in men for whom sexual activity is inadvisable (e.g. patients with severe cardiovascular disorders such as unstable angina or severe cardiac failure.

The safety of vardenafil has not been studied in the following sub-groups of patients and its use is therefore contraindicated until further information is available:

  • severe hepatic impairment (Child-Pugh C)
  • end stage renal disease requiring dialysis
  • hypotension (blood pressure <90/50 mmHg)
  • recent history of stroke or myocardial infarction (within the last 6 months)
  • unstable angina and known hereditary retinal degenerative disorders such as retinitis pigmentosa
  • CYP3A4 inhibitors ketoconazole and itraconazole (oral form) in men older than 75 years
  • HIV protease inhibitors such as ritonavir and indinavir
  • PDE5 inhibitors, including vardenafil, with guanylate cyclase stimulators, such as riociguat

Some medicines may cause problems, especially these:

  • Nitrates, medicines for angina, or nitric oxide donors, such as amyl nitrite. Taking these medicines with Valif Oral Jelly 20 mg could seriously affect your blood pressure
  • Medicine for the treatment of arrhythmias, such as quinidine, procainamide, amiodarone or sotalol
  • Ketoconazole or itraconazole, antifungal medicines
  • Erythromycin or clarithromycin, macrolide antibiotics
  • Alpha-blockers, a type of medicine used to treat high blood pressure and enlargement of the prostate (as benign prostatic hyperplasia)

Don't drink grapefruit juice when you use Valif Oral Jelly 20 mg. It can interfere with the usual effect of the medicine. Alcoholic drink can make erection difficulties worse.

How it works

Vardenafil is an oral therapy for the improvement of erectile function in men with erectile dysfunction.

In the natural setting, i.e. with sexual stimulation, it restores impaired erectile function by increasing blood flow to the penis.

Penile erection is a haemodynamic process. During sexual stimulation, nitric oxide is released. It activates the enzyme guanylate cyclase, resulting in an increased level of cyclic guanosine monophosphate (cGMP) in the corpus cavernosum. This in turn results in smooth muscle relaxation, allowing increased inflow of blood into the penis.The level of cGMP is regulated by the rate of synthesis via guanylate cyclase and by the rate of degradation via cGMP hydrolysing phosphodiesterases (PDEs).

Vardenafil is a potent and selective inhibitor of the cGMP specific phosphodiesterase type 5 (PDE5), the most prominent PDE in the human corpus cavernosum. Vardenafil potently enhances the effect of endogenous nitric oxide in the corpus cavernosum by inhibiting PDE5. When nitric oxide is released in response to sexual stimulation, inhibition of PDE5 by vardenafil results in increased corpus cavernosum levels of cGMP. Sexual stimulation is therefore required for vardenafil to produce its beneficial therapeutic effects.

Tips

Do not take more than one dose of Valif Oral Jelly 20 mg per day.

It is recommended that tablets be taken with at least one full glass of water.

Do not consume grapefruit or grapefruit products while taking Valif Oral Jelly 20 mg.

Always talk to your doctor before taking ANY medication for erectile dysfunction. Some erectile dysfunction medications are not suitable for men with certain medical conditions, and your doctor can advise you on an alternative.

Call your doctor immediately if you experience dizziness or feel nauseous during sexual activity, or develop pain, numbness, tingling in your chest, arms, neck, or jaw.

Seek emergency treatment if an erection lasts more than 4 hours.

Do not take Valif Oral Jelly 20 mg if you are also taking a nitrate drug (such as nitroglycerin or isosorbide dinitrate/mononitrate) for chest pain or heart problems. Do not take Valif Oral Jelly 20 mg with recreational drugs such as "poppers". Taking these substances together can cause a sudden and dangerous drop in blood pressure.

Always buy your medications from a reputable supplier as there are a lot of counterfeit products on the market.

Certain lifestyle and psychological factors, such as smoking, drinking, and stress, can impact on erectile dysfunction. Eliminating these may also help improve the erection, for example:

  • staying more active
  • losing weight if you are overweight
  • reducing alcohol intake
  • stopping smoking and reducing stress

Never take other erectile dysfunction medications at the same time as taking Valif Oral Jelly 20 mg.

Seek urgent medical advice if an allergic reaction, sudden vision loss or painful erections lasting more than four hours occur.

Drinking alcohol after taking Valif Oral Jelly 20 mg may lower blood pressure and decrease sexual performance.
